Abstract
The Immunization Program releases school immunization audit reports. Local Public Health Agencies (LPHAs) conduct audits of immunization records for students enrolled in public, private and parochial students in kindergarten through 12th grade each year. Assessments are conducted to ensure attendees receive required immunizations and have a valid Certificate of Immunization, Certificate of Immunization Exemption, or Provisional Certificate of Immunization.
