Any person claiming to be aggrieved by a discriminatory or unfair practice may, in person or by an attorney, make, sign, and file with the Commission a verified, written complaint, which shall state the name and address of the person, employer, employment agency, or labor organization alleged to have committed the discriminatory or unfair practice of which complained, shall set forth the particulars thereof, and shall contain such other information as may be required by the Commission.

A complaint may be filed in person or by mail, fax, or courier service.

Contents of complaint. Each complaint of discrimination should contain the following:
- The full name, address and telephone number, if any, of the person making the complaint;
- The full name and address of each respondent;
- A clear and concise statement of the facts, including pertinent dates, if known, constituting each alleged unfair or discriminatory practice;
- If known and if employment discrimination is alleged, the approximate number of employees of a respondent employer.

The date a complaint of discrimination is filed is the date the complaint is received by the Commission.
If the complaint is filed by FAX and is received outside regular office hours (8 a.m. - 4:30 p.m. Monday - Friday), the date of filing is the next regular business day. Transmissions received prior to office hours on a regular business day are filed on that day.
